Envelope Leakage Test Report Forms for You

 

The BOAF Board of Directors are proud to announce the creation of Envelope Leakage Test Report forms that can be submitted to the local jurisdiction and used to indicate compliance with the minimum requirements of the code, regarding Blower Door Envelope Leakage and Duct Leakage.

The forms are meant to offer a consistent means throughout the state for testing agencies and jurisdictions to keep this process simple. Testing agencies will have their own forms, jurisdictions will create their own forms, and other forms can be found online to use. Many forms require more information than is needed. The forms created by the BOAF Code Development Committee are an effort to keep it simple and to include the minimum information required to indicate compliance. We believe the forms will help with consistency and are easy for the jurisdictions staff to review and approve. Companies and jurisdictions may also add their logos to the forms to personalize them. 

The forms may be downloaded from the members-only BOAF Document Exchange (click here), a PDF-fillable form and a blank form that may be reproduced.
